The Communicable Disease Investigations and Outbreaks team requires self-directed individuals to work within a collaborative environment to deliver services in accordance with the Ontario Public Health Standards (OPHS) for Communicable Disease Control.
Team Specific Responsibilities:
- Case, contact & outbreak management for reports of communicable diseases of Public Health Significance (DOPHS)
- Investigate and conduct outbreak management for gastrointestinal and respiratory outbreaks in Acute Care/Long Term Care Homes/Retirement Homes/congregate living settings and provide direction for infection prevention and outbreak control measures
- Develop, foster and facilitate inter-sectoral partnerships and networks using team building, negotiation, conflict management and group facilitation
- Provide health information, teaching and counselling to clients as part of case and contact management of DOPHS
- Counsel and educate on health promotion, disease prevention and public health interventions including immunization, prophylaxis and/or harm reduction
- Develop and maintain linkages and work collaboratively with health professionals, agencies and other external partners (e.g. physicians, local hospitals and community health agencies, Long Term Care, Public Health Ontario, Immigration Refugee Citizenship Canada)
- Work collaboratively with team members, demonstrating flexibility, adaptability, problem solving, effective communication and continuous quality improvement, to meet the operational needs of the program and our clients
- Assist supervisor in program development including, but not limited to, developing procedures, work instructions and support materials related to case, contact and outbreak management
- Participate in resource development, and training in preparedness to address new or emerging infectious diseases
- Support Communicable Diseases Urgent Responses as required
- Documentation and data entry in electronic medical records and sharepoint documentation system and reporting to the MOHLTC through iPHIS
Team Specific Requirements:
- Minimum one year of case and contact management of non-COVID disease of public health significance (DOPHS) and 1 year of outbreak management experience in congregate living settings (Long Term Care Homes, Retirement Homes, Acute Care Settings and other Congregate Living Settings)
- Experience supporting non-COVID Communicable Disease (CD) urgent responses an asset
- Experience with Communicable Disease (CD) on-call program is an asset
- Experience working with epi curves an asset
- Completion of an IPAC course or participation in an IPAC committee
- Resource development such as policy and procedure writing, creation of tools, etc an asset
- Leading a project or working group an asset
- Strong conflict resolution and de-escalation skills
- Effective written and oral communication skills
- Training or mentorship experience
- Public speaking or presentation skills an asset
What you will do in the role:
- Collaborate with internal and external partners to identify and address public health issues and service gaps to achieve improved health outcomes
- Demonstrate effective oral and written communication skills via the telephone and email with internal and external stakeholders, including clients, health care providers and community facilities
- Apply a holistic approach to assess the health status and functional competence of clients with consideration of the impact of the determinants of health
- Identify and analyze how clients' culture, attitudes, beliefs and feelings about health influence interventions
- Establish mutually determined goals with client, including outcome indicators and evaluative measures
- Facilitate client engagement in identifying and defining health issues using community development and community assessment skills
- Participate in research, quality assurance initiatives, and mentorship opportunities as appropriate
- Apply knowledge regarding concepts of population health status, inequities in health, determinants of health, and strategies for health protection and promotion to guide practice
- Demonstrate culturally sensitive approaches and strategies with diverse populations
- Perform other duties as assigned which support the organizational goals
- Respond to public health emergencies, significant events and incidents as required by the Health Protection and Promotion Act and Emergency Standards
- Adhere to public health and nursing ethical standards and principles
- Fulfill legal and professional obligations and responsibilities in accordance with relevant legislation and departmental policies, procedures and medical directives/acts as an ambassador for the Region of Peel, Public Health Services and Public Health Nursing
What the role requires:
- A Baccalaureate Degree in Nursing or equivalent recognized by a Canadian University
- Current license to practice as a Registered Nurse by the College of Nurses of Ontario
- Current certification in BLS
- Minimum one year of case and contact management of non-COVID disease of public health significance (DOPHS) or minimum one year of outbreak management experience in congregate living settings (Long Term Care Homes, Retirement Homes, Acute Care Settings and other Congregate Living Settings)
Skills/Abilities:

- Knowledge of Peel’s diverse community and associated health impacts
Nice to have:
- Experience in public health nursing
- Experience with policy and procedure development and update
- Experience with CD On-Call and/or urgent response
- Experience with infection prevention and control
- Experience with iPHIS platform
Work Mode & Job Location: In this role, you’ll benefit from the flexibility of a hybrid work arrangement that prioritizes meaningful in-person connection. A minimum of two days per week on-site is expected, with additional in-person presence as operational needs evolve. This arrangement allows for a blend of in person and hybrid work that adapts to support both organizational priorities and employee flexibility and is subject to change over time.
It's important to note that your off-site work location must be within the province of Ontario. Hours of work: Monday to Friday 8:30AM - 8:30PM availability is required, as per the Collective Agreement.
