Hours of work: 35 hours per week. Monday to Friday. May require evening, weekend and statutory holidays availability based on operational need
Salary Range: $46.00 to $54.59 per hour as per the collective agreement

Familiy Health PHNs collaborate with other family health program areas, community and hospital partners to support a smooth transition for all families through pregnancy, birth and early parenting. They contribute to program planning, evaluation and the development of program innovations that facilitate integration between family health and other programs, and client centred systems of support. Family Health PHNs lead and contribute to the maintenance and upkeep of program information, resources, client facing teaching materials, and online content. They contribute to activities and outcomes in the public health strategic plan, specifically enabling active living and healthy eating, promoting mental well-being, and advancing health equity. Family Health PHNs lead, facilitate and participate in meetings and groups at the team, work group and community level. Proficiency with Microsoft, booking, and information management systems
Reproductive Health Team Specific Responsibilities
- Provide expertise related to preconception, prenatal and reproductive health
- Provide direct services within community settings
- Collaborate with other family health program areas, community and hospital partners to support a smooth transition for all families through pregnancy, birth and early parenting
- Contribute to program planning, evaluation and the development of program innovations that facilitate integration between family health and other programs, and client centred systems of support
- Lead and contribute to the maintenance and upkeep of program information, resources, client facing teaching materials, and online content
- Contribute to activities and outcomes in the public health strategic plan. Specifically Enabling Active Living and Healthy Eating, Promoting Mental Well-being, and Advancing Health Equity
- Contribute to a positive workplace culture by working effectively on an individual basis and in team environments
- Lead, facilitate and participate in meetings and groups at the team, work group and community level
- Proficiency with Microsoft, booking, and information management systems
Nutrition and Physical Activity (NAPA) Team Specific Responsibilities
- Demonstrate experience using project management, communication, and facilitation skills
- Demonstrate analytical and problem-solving skills, along with effective organizational and time management skills
- Establish and maintain relationships with internal and external partners
- Develop, implement, and evaluate programs, policies and processes with available evidence and data to support Nutrition and Physical Activity programming in Child Care and EarlyON centres across the region
- Support capacity building of professional staff focused on physical activity, physical literacy, and nutrition
- Work effectively both independently and as part of a team
- Current CPR certification, Level C or HCP, is a requirement for Public Health Nurses in the Family Health Division
- Candidates may be required to travel within Peel region based on operational needs (e.g., Childcare and EarlyON centres)
What you will do…
- Apply knowledge regarding concepts of population health status, inequities in health, determinants of health, and strategies for health protection and promotion to guide practice
- Utilize research and evidence and apply critical appraisal skills to inform policies, programs and practice
- Provide health information, teaching and counselling to clients
- Use a holistic approach to assess the health status and functional competence of clients with consideration of the impact of the determinants of health
- Identify and analyze how clients' culture, attitudes, beliefs and feelings about health influence interventions
- Establish mutually determined goals with client, including outcome indicators and evaluative measures
- Facilitate client engagement in identifying and defining health issues using community development and community assessment skills
- Participate in the planning, developing, implementation and evaluation of community wide and site-specific public health strategies using Evidence Informed Decision Making (EIDM)
- Advocate for and contribute to the development of healthy public policy
- Collaborate with partners to identify and address public health issues and service gaps in order to achieve improved health outcomes
- Develop, foster and facilitate inter-sectoral partnerships and networks using team building, negotiation, conflict management and group facilitation
- Demonstrate culturally sensitive approaches and strategies with diverse populations
- Communicate effectively with internal and external stakeholders
- Promote awareness of the Region of Peel and Public Health services
- Participate in research, quality assurance initiatives, and mentor-ship opportunities as appropriate
- Adhere to public health and nursing ethical standards and principles
- Fulfill legal and professional obligations and responsibilities in accordance with relevant legislation and departmental policies, procedures and medical directives/acts as an ambassador for the Region of Peel, Public Health Services and Public Health Nursing
- Perform other duties as assigned which support the organizational goals
- Respond to public health emergencies, significant events and incidents as required by the Health Protection and Promotion Act and Emergency Standards
What the role requires:
- A Baccalaureate Degree in Nursing or equivalent recognized by a Canadian University
- Current license to practice as a Registered Nurse by the College of Nurses of Ontario
- Current CPR certification, Level C or HCP, is a requirement for Public Health Nurses in Family Health
- Experience in public health or community health nursing is preferred

Work Mode & Job Location: In this role, you’ll benefit from the flexibility of a hybrid work arrangement that prioritizes meaningful in-person connection. A minimum of two days per week at7120 Hurontario Street is expected, with additional in-person presence as operational needs evolve. This arrangement allows for a blend of in person and hybrid work that adapts to support both organizational priorities and employee flexibility and is subject to change over time. Please note you may be required to travel to various community sites in Peel Region (Mississauga,Brampton & Caledon).
