Program Overview – Youth Outreach Team, Community CareThe Youth Outreach Team is a collaborative project between Michael Garron Hospital and WoodGreen Community Services, funded by the Slaight Family Foundation. This team provides outreach services to aid and uplift youth from the ages of 16-29 with complex mental health and/or substance use concerns in East Toronto. This position works on behalf of WoodGreen Community Services and is responsible for launching, operations, evaluation, and growth of a new community-based service for youth.What You Will Do- Support program leadership team with recruitment, interviewing and retention of staff as well as supervise and manage any performance issues on the teams.
- Provide/organize orientation and training for new Youth Outreach Team staff, students and volunteers.
- Monitor staff performance targets and data quality by regularly reviewing documentation in client management systems, calls reports, statistics, and consultation with staff.
- Coordinate Youth Outreach Team program and provide operational oversight.
- Develop and/or update program materials to ensure compliance with organizational, funder-specific, and accreditation standards and requirements and ensure these specific standards and targets are met.
- Work with referral partners to identify and engage clients and provide support in making referrals to the Youth Outreach Team.
- Follow up with referral source(s) to assess program eligibility, confirm receipt of referral, obtain additional information to support client care, and/or offer information on alternative sources of support if the client is deemed ineligible for the program.
- Develop relationships with other service providers to make seamless, effective referrals and service connections, and coordinate referral assignment and distribution to ensure flow and service quality.
- In conjunction with WoodGreen and MGH’s Communications department, implement ongoing promotion/awareness building of the program.
- Identify and implement mechanisms for community and client input to support quality improvement measures and foster ongoing enhancement of the program.
- Identify and actively participate in existing and new partnership initiatives to enhance the program and promote its reach and sustainability, as required.
- Support development of funding proposals, as required.
- Participate in system-level networks on behalf of WoodGreen and MGH.
- Complete statistical reports and submissions and maintain all records and forms regarding visits and program/activity enrollment, using a client information database in a timely manner.
- In conjunction with the program leadership team, support program development and evaluation activities and implement new services and initiatives that build on and enhance the objectives of the program.
- Provide clinical supervision to the Youth Outreach Team staff
- Organize and facilitate team meetings.
- In conjunction with Volunteer Services recruit and train volunteers for the program, as needed
- Ensure compliance with organizational policies, Collective Bargaining Agreement, and legislative requirements.
- Engage in training and education to upgrade skills as directly by WoodGreen/MGH to fulfill the responsibilities of the position.
- Keep current about the range of services available within the organization and in other agencies.
- Take part in team, unit and management meetings for the purposes of ensuring coordinated and integrative programming, and resource sharing.
- Participate in regular supervisory sessions and annual performance appraisals with the Manager.
- Be familiar with the organization’s policies and procedures and the site-specific emergency procedures.
- Represent the unit on internal staff committees, inter-agency initiatives as directed by the Unit Director/ Manager.
- Perform other related duties as required.
What You Bring to the Team- Masters of Social Work (M.S.W.) or post-graduate degree in relevant field.
- Member in good standing with a Professional College in Ontario.
- Minimum of 5 years of clinical experience in mental health and/or addictions and preferably the child and youth sector.
- 2 years progressive leadership experience, preferably in a unionized environment.

What Will Set You Apart |
- Demonstrated ability in supporting youth who have complex mental health and/or addiction issues, history of trauma, violence/abuse, chronic physical health issues, etc.
- Comprehensive understanding and knowledge of mental health, addictions, children & youth, health sector, and community resources.
- Knowledge of, experience with, and demonstrated ability in providing therapeutic interventions, such as Narrative Therapy, Solution-Focused Therapy, Cognitive-Behavioral Therapy, Dialectical Behavior Therapy, Motivational Interviewing, Trauma-Informed approaches, etc.
- Proven working knowledge of theories of addiction, evidence-based treatment models and best practice approaches.
- Ability to demonstrate an anti-oppression approach to practice and work from a client-centered, anti-racism, harm reduction, and trauma informed approach is required.
- Demonstrate knowledge of issues affecting marginalized communities, policies, legislation, programs, and other issues related to scope of practice and social determinants of health.
- Experience working with diverse and marginalized communities, including ethno-cultural and religious groups, LGBTQSS+ youth and youth adults, those impacted by poverty and those involved in youth justice.
- Minimum of 2 years’ clinical experience working with youth and young adults aged 16- 29.
