Program Overview – Seniors’ Mental Health & Addictions
This position works on behalf of WoodGreen Community Services, which is the lead agency for the Seniors Crisis Services Initiative, encompassing Toronto Seniors Helpline and the Crisis Outreach Service for Seniors (COSS). This is a liaison role between WoodGreen/Toronto Seniors Helpline/COSS and the Toronto Community Crisis Services team through Gerstein Crisis Centre. The philosophy of this work is in keeping with WoodGreen’s mission to enhance self-sufficiency, promote well-being and reduce poverty through innovative solutions to critical social needs.
What You Will Do
- Facilitate coordination and connection between COSS (Crisis Outreach Service for Seniors), TSH (Toronto Seniors Helpline) and TCCS (Toronto Community Crisis Services)
- Establish contacts and clear lines of communication between COSS and TCCS leadership
- Establish regular meetings between management of both crisis programs
- Clearly delineate the differences between the programs related to eligibility and service offerings that can be used internally and externally
- Create and implement a communication plan for various stakeholders (including frontline staff in the programs, management, the public, clients of both programs, etc. etc.) to ensure that “no wrong door” is in place, and that whether people call TSH or TCCS/211/911, they receive the service and support they need
- Create formalized and written guidelines, pathways and protocols for TSH/COSS staff which will clearly outline when and how to refer to TCCS for support and what that support will look like, ensuring there is seamless, smooth connection
- In collaboration with TCCS, create formalized and written guidelines, pathways and protocols for TCCS for when and how to refer to TSH/COSS; ensuring there is seamless, smooth connection
- Create formalized and written guidelines, pathways and protocols for TCCS staff related to having consultation for clients with COSS Crisis supervisor or other COSS staff related to how to best support seniors in crisis being seen by TCCS in real-time
- Explore possible data sharing agreements and/or having access to client management systems both for WG/COSS/TSH and TCCS/Gerstein in order to better facilitate communication between the teams, ensuring there is no overlap of service and that collaboration/coordination are occurring
- Coordinate training and education support for TCCS staff on seniors’ specific services and best practices for supporting seniors and older adults in crisis
- Based on learnings and exposure to both teams, create and establish guidelines for best practices related to supporting seniors and adults in crisis
- Identify opportunities for mutual learning, consultation and connection between frontline staff on TCCS and COSS, including joint trainings and possible community of practice
- Ensure that an anti-oppression, community development and harm reduction principles are achieved in the program
- Perform other related duties as required
What You Bring to the Team
- Master’s degree in social work, Project Management, Psychology or another related field
- Demonstrated ability in supporting adults and seniors who have complex mental health and/or addiction issues, history of trauma, violence/abuse, chronic physical health issues, etc.
- Experience working with and supporting diverse and marginalized communities.
- Comprehensive understanding and knowledge of the mental health, addictions, geriatrics, health sector, and community resources.
- Demonstrate knowledge of issues affecting marginalized communities, policies, legislation, programs, and other issues related to scope of practice and social determinants of health.
- Experience in providing project management support in the community/health care sector
- Experience in facilitating/developing inter-agency partnerships
- Experience in program development
- Demonstrated experience in developing policies, procedures and guidelines
- Excellent verbal and written communication skills
